Air pollution has become one of the most serious but under-addressed health challenges in African cities. Rapid urbanisation, weak infrastructure, and growing dependence on fossil fuels are exposing millions of people daily to unsafe air.

Globally, air pollution is linked to about 1.1 million deaths annually in Africa, driven by both outdoor and household exposure. In many urban centres, fine particulate matter (PM2.5) levels are far above safe limits. While the World Health Organization recommends an annual average of 5 µg/m³, many African cities regularly record between 30 and 80 µg/m³, depending on traffic, industry, and seasonal dust.

Cities such as Lagos, Nairobi, Accra, Cairo, and Johannesburg are among the most affected, with pollution levels rising alongside population growth and vehicle use.

Major sources of urban air pollution

Transport remains one of the biggest contributors to poor air quality. In several African megacities, road traffic is responsible for about one third of PM2.5 emissions, driven by congestion, ageing vehicles, and weak fuel regulation.

Other major sources include diesel generators, which are widely used due to unstable electricity supply, as well as industrial emissions and open waste burning. Studies show that industrial activities can contribute between 11% and 37% of PM2.5 pollution in some urban environments. In many communities, burning of refuse adds toxic smoke directly into residential air.

Health impact on the population

The health consequences are severe. PM2.5 particles are small enough to penetrate deep into the lungs and enter the bloodstream, making long-term exposure particularly dangerous.

Air pollution is linked to asthma, chronic obstructive pulmonary disease (COPD), lung infections, heart disease, and stroke. Health data shows that air pollution causes about 155 deaths per 100,000 people in Africa, nearly double the global average.

Hospitals in major cities report increasing respiratory cases, especially among children and older adults. Doctors often see spikes in asthma attacks and breathing difficulties during periods of heavy traffic, industrial activity, or dry-season dust.

Indoor pollution and hidden exposure

Indoor air pollution is another major contributor to respiratory disease. Millions of households still rely on charcoal, firewood, and kerosene for cooking. In poorly ventilated homes, smoke exposure can reach levels several times higher than safe outdoor limits.

Women and children are most affected because they spend more time around cooking areas. Over time, this constant exposure increases the risk of chronic respiratory illness and reduces overall lung function.

As African cities expand, pollution sources are moving closer to residential areas, increasing daily exposure. Without stronger regulation, cleaner energy alternatives, and improved urban planning, respiratory diseases are likely to continue rising.

Urban air pollution is no longer just an environmental issue. It is now a public health crisis shaping the future of respiratory health across Africa.
